Our office will file insurance for you. In order to provide this service, we must have your complete insurance information including address and policy numbers. It is important that you bring your insurance information with you at the time of your first appointment. Should your insurance change during the course of the treatment with our office, please be sure and notify our front desk.

*Some insurance companies don’t require being a preferred provider please call your insurance company if you have any questions about benefits prior to your visit.

Medicare

Dr. Hungerford is a participating physician with the Medicare program. We will be happy to submit claims to Medicare for you. Be sure to bring your Medicare card with you.

Workman’s Compensation

We will gladly bill your workmen’s compensation carrier for our services if your injury is work related. Bring your company’s name, address, claim numbers, and claim adjuster handling your claim. Check with your employer for this information before your appointment. If workmen’s compensation denies your claim or is contesting your case, we will ask you pay your bill in full the day of your appointment.

No Insurance

If you do not have insurance, you will be asked to pay in full at the time of your visit.

Auto Accident

We will gladly bill your auto insurance for our services if your injury is related to an automobile accident. Please bring all your insurance information the day of your first appointment.

Payment Policy

Payment is due at the time of the service.
